The Independent Complaints Reviewer is opposed to discrimination on any grounds. This office is committed to ensuring that our service meets the needs of all sections of the community. We also want to know if anyone is suffering any prejudice because of their race, nationality, colour, gender, sexual orientation, disability or age.
It will help us to achieve these aims if you complete all or parts of this questionnaire and return it to us. This will help us to improve our service and that of the organisation about which you are complaining. Your answers will be collated by our Diversity Officer and used for statistical purposes only. Your answers will not be seen by any Investigation Officer handling your complaint and will be completely confidential.
